Add a declaration to a form

Adding a declaration at the end of a form is an essential step in ensuring that users confirm they have understood and agreed to the information provided. This feature is especially useful in situations where compliance, legal acknowledgement, or user consent is required before submission. For example, if you're collecting sensitive data or asking users to agree to terms and conditions, a declaration serves as a clear confirmation of their agreement.

⚠️ Important: If the form already exists, a new version must be created before being able to complete the below steps.

To add a declaration to a form in draft status, follow the below steps.

  1. Switch to admin view.

  2. Click Content, then click All content.

  3. Click Manage, then search for and select the relevant form.

  4. Click Layout, then click Add Section.

  5. Click Form, then enter a name for the section. Click Confirm

  6. Select the black circle for the radio button option.

  7. Use the radio button fields to configure the declaration.

  8. Click Save.

The new version must be published before it's visible to users.
