Add a declaration to a form

Guidance on how to add a declaration to the end of a form.

You can add a declaration at the end of a form to confirm that users understand and agree to the information you’ve provided. This helps you capture clear consent before the form is submitted.

Declarations are useful when you need users to acknowledge legal terms, confirm compliance requirements, or agree to how their data will be used. They’re also helpful when collecting sensitive information or when a formal confirmation is required.

⚠️ Important: If the form already exists, a new version must be created before being able to complete the below steps.

To add a declaration to a form in draft status, follow the below steps.

  1. In admin view, click Tools, then select Form Templates.

  2. Search for and select the relevant unpublished (draft) version of the template.

  3. Click Layout, then click Add Section.

  4. Click Form, then enter a name for the section. Click Confirm

  5. Select the black circle for the radio button option.

  6. Use the radio button fields to configure the declaration.

  7. Click Save.

📌Note: The new version must be published before it's visible to users.
